Cardiogenic shock (CS) is characterised by tissue hypoxia as a result of circulatory failure arising from inadequate cardiac output and is commonly a complication of acute myocardial infarction (AMI). Despite improvement in reperfusion strategies for AMI, the survival among patients with CS remains poor. While mechanical circulatory support (MCS) technologies in AMI-CS offer promise, they have not translated to consistent improvements in patient survival, which may reflect an inability to rec...
Older patients represent the most rapidly growing age group presenting with kidney failure. Despite this high incidence, the rate of progression to kidney failure tends to be slower in older individuals, and the competing risk of death before the development of kidney failure is a more significant consideration in older patients compared with younger counterparts. Incorporating these concepts of risk is challenging in shared decision-making discussions between clinicians, older patients, and ...
Background: People receiving treatment for breast cancer often report sleep disturbance. This systematic review explored the prevalence and impact of breast cancer treatment on sleep disturbance. Methods: The Medline, Embase, CINAHL Plus with full text, Psych INFO, Cochrane Library/Central Register of Controlled Trials, and Scopus databases were searched up to December 2024. Eligible studies recruited people undergoing breast cancer treatment and reported the impact of treatment on their slee...
Patients with newly diagnosed multiple myeloma and considered transplant ineligible (TIE) because of age, frailty and/or comorbidities now have access to highly effective therapies that can achieve deep and/or durable remission. TIE patients are a highly heterogeneous population whose biological and chronological age can vary substantially. The treatment of these patients can be challenging in clinical practice and requires a frailty-adapted, individualised approach with an emphasis on treatm...
Cancer survivors experience a range of side effects during and after treatment. There is a need for a rigorous synthesis of the most recent and best available evidence on the role of nutritional supplements for supportive care in cancer, to inform shared decision-making. We searched 5 databases for umbrella reviews, meta-analyses and systematic reviews on nutritional supplements for supportive cancer care, excluding studies on pain, anxiety and depression, which are covered in recent guidelin...
Background: The management of persistent symptoms for long COVID (eg, fatigue, concentration difficulties, sleep difficulties, loss of appetite and taste, depression, and anxiety) has not been widely studied among adolescents and young adults (AYA). This systematic review and meta-analysis aimed to synthesise and review evidence on the effectiveness of non-pharmacological interventions for AYA aged 13?25 years, presenting with long COVID symptoms. Methods: A systematic literature search was c...
Background: Medical extended reality projects a 3D digital image onto live surface anatomy via a wearable device while allowing operators to interact with the real world. We have developed a 3D hologram of the thoracic spine from CT images of a soft embalmed Thiel cadaver and visualised spinal anatomy through a Microsoft Hololens2 headset. The primary objective of this study was to test whether thoracic epidural training performance on the cadaver was improved using extended reality compared ...
Background: Radiotherapy is a vital component of cancer care, yet access is limited. Global estimates often overlook cancer stage variability across countries with different income levels. This study assesses the supply?demand gap for megavoltage radiotherapy machines (MVMs) from 2012 to 2022 and projects the survival and local control benefits achievable by meeting optimal radiotherapy needs by 2050. Methods: Global cancer data were from GLOBOCAN 2022. A validated, stage-adjusted radiotherap...
Objectives: To understand perspectives of primary care professionals and trainee primary care professionals on discussions with higher weight patients focused on weight management support, particularly the barriers and facilitators to these discussions. Methods: Qualified primary care professionals and trainee primary care professionals (N = 91) at varying career stages within Australia completed an online survey (as part of a larger study) with a series of open-ended questions about their ex...
We describe a novel "Sleeve Colonoscopy" technique, a pragmatic approach to preserving surgical sterility during cases where on-table colonoscopy or enteroscopy is required. The sleeve of a readily available surgical gown is used to create a barrier between the sterile surgical field and the endoscopy equipment path via a colotomy or enterotomy. 2025 International Society of Surgery/Socit Internationale de Chirurgie (ISS/SIC).
Mental health consumers experience challenges in accessing regular physical healthcare. Staff working in mental health settings are well-positioned to address the issue; however, they often face significant barriers in delivering physical healthcare. The primary aim of this study was to identify important factors that support mental health staff to address the physical health of consumers; the secondary aim was to evaluate the utility of using a modified nominal group technique (NGT). Four ma...
Background Melanoma patients with pre-existing autoimmune diseases (AIDs) have been excluded from clinical trials of immune checkpoint inhibitors (ICIs), due to a risk of flare and immune-related adverse effects (irAEs). Materials and methods A comprehensive literature search of Medline, Embase, CINAHL and Scopus was carried out. Studies of melanoma patients with pre-existing AIDs were included. Two reviewers used the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA)...
Background: Allergic rhinitis (AR) is a highly prevalent condition associated with significant morbidity globally. Few recent studies have detailed the experiences of sufferers and explored their perspectives of treatment options. Allergen immunotherapy (AIT) is an effective treatment option that remains underused in eligible patient populations. Objective: We sought to describe patient perspectives of AR and treatment options including AIT. Methods: Twenty-five semistructured interviews were...
Desflurane is a potent and expensive greenhouse gas. Reducing its use is a global priority. This anaesthetist-led quality improvement project involved educational, motivational and system-change initiatives implemented in the anaesthesia department of Bankstown-Lidcombe Hospital (BLH) (September 2021?March 2024), with the aim of reducing desflurane consumption. A quasi-experimental interrupted time series design with control site was employed to estimate changes in usage, greenhouse gas emiss...
Background: Presence of RAS mutations in mCRC are negatively correlated with OS. NGS enables detailed information regarding the heterogeneity of RAS mutations including specific amino acid changes as well as variant allele frequency (VAF). Recently, VAF has been postulated to have a role in identifying resistance to treatments in multiple solid tumours but its role as a prognostic marker of OS in mCRC is not established. Methods: This was a multicentre, retrospective cohort study investigatin...
Background Keloids are aggressive fibroproliferative scars extending beyond wound margins, with high recurrence despite multiple treatment modalities. Combining surgical excision with radiotherapy reduces recurrence, but the optimal timing of radiotherapy remains controversial. Aim To evaluate the effectiveness and optimal timing of post-excisional radiotherapy in preventing keloid recurrence. Methods A systematic review was conducted using Scopus, Web of Science, PubMed, and Cochrane databas...
Background/Objectives: Emergency arterial embolization is a life-saving procedure typically performed via femoral access. This study evaluated the safety and efficacy of immediate femoral sheath removal following emergency embolization. Methods: A retrospective cohort study was conducted at a Level 1 trauma center (January 2022-May 2025). Adult patients undergoing emergency embolization with immediate sheath removal were included. Endpoints were reintervention (repeat embolization within 7 da...
