The range of biologic therapies currently available for treating inflammatory bowel disease (IBD) include anti-tumor necrosis factor (anti-TNF) agents, infliximab, adalimumab, certolizumab, and golimumab; one anti-adhesion molecule, vedolizumab; and one interleukin (IL) 12/IL-23 antagonist, ustekinumab. The foundation of selecting the optimal treatment for a patient with IBD rests in individualized assessment of a patient?s risk-to-benefit ratio. 2022 by Taylor & Francis Group.
Introduction: Local Australian guidelines for the optimal management of stage III unresectable non-small cell lung cancer (NSCLC) are lacking. The American Society of Clinical Oncology (ASCO) guidelines recommend consolidation durvalumab for all patients with unresectable stage III NSCLC, irrespective of their PD-L1 expression or driver mutation status. The European Society of Medical Oncology (ESMO) differs, with consolidation durvalumab only recommended in those patients whose tumours expre...
Background: The European Resuscitation Council 2021 guidelines for haemodynamic monitoring and management during post-resuscitation care from cardiac arrest call for an individualised approach to therapeutic interventions. Combining the cardiac function and venous return curves with the inclusion of the mean systemic filling pressure enables a physiological illustration of intravascular volume, vasoconstriction and inotropy. An analogue mean systemic filling pressure (Pmsa) may be calculated ...
The development and market emergence of vaginal sensors have begun to demonstrate their impact on women?s healthcare. Until recently, in limited cases, these sensors have exhibited their capabilities in diagnosing and monitoring disorders of the vaginal tract during different stages of women?s lives. This Perspective is a compilation of what has been accomplished so far in the landscape of vaginal sensors. The text explores the diverse types of vaginal sensor technologies, their applications,...
Study objective: We previously designed and validated a virtual reality-based simulator to help train novices in ultrasound-guided needling skills necessary for safe and competent ultrasound-guided regional anaesthesia. This study was designed to compare the performance and error rates of novices trained by a human faculty aided with the assistance of this virtual reality simulator (virtual reality-assisted training), versus novices trained wholly by humans (conventional training). Design, se...
The heterogeneity of autism spectrum disorder (ASD) clinically and aetiologically hinders intervention matching and prediction of outcomes. This study investigated if the behavioural, sensory, and perinatal factor profiles of autistic children could be used to identify distinct subgroups. Participants on the autism spectrum aged 2 to 17 years and their families were sourced via the Australian Autism Biobank (AAB). Latent class analysis was used to identify subgroups within this cohort, utilis...
Background: The Sydney Facial Nerve Service is a multidisciplinary team that provides management of patients with facial nerve disorders, historically as an in-person format. The coronavirus disease 2019 (COVID-19) pandemic resulted in a shift to a virtual format due to infection control precautions. This study aims to evaluate the acceptability of a telehealth format for patients and clinicians in the multidisciplinary care of facial nerve palsy. Methods: The questionnaires were distributed ...
Aim: To conduct a child and family health nursing service redesign to improve pathways of access, response and outcomes for all families with children aged 0?5 years. Design: The study was conducted as an iterative, mixed-method study of the process and impact of the service redesign, informed by a participatory action research paradigm and the NSW Agency for Clinical Innovation process for developing a model of care. Methods: Diagnostic, solution design, implementation and sustainability pha...
Purpose: To investigate massage therapists? attitudes, consultation processes, and confidence to respond to situations involving domestic and family violence (DFV) in clinical practice. Methods: An online survey-based study was conducted and Australian massage therapists, 18years of age or over were recruited. The survey included 64 questions in 3 sections: 1) Demographics, 2) Recognize Family and Domestic Violence, and 3) Respond to domestic and family violence with 4 subsections; i) disclo...
Currently, there are no resources to support culturally and linguistically diverse (CALD) women with breast cancer to make decisions about undergoing breast reconstruction (BR). This study evaluated the usability and acceptability of decision aids (DAs) for Vietnamese- and Arabic-speaking women. This two-phase qualitative recruited Vietnamese- (Phase 1) and Arabic-speaking (Phase 2) adult (age ? 18 years) women who were diagnosed with breast cancer and could read Vietnamese/Arabic. Women part...
Background: Autistic children are prone to experience heightened levels of distress and physiological reactivity to a range of sensory, social, and emotional stimuli. In line with this, multiple studies have demonstrated that autistic children have higher acute cortisol stress responses to adverse or threatening stimuli and altered cortisol awakening responses. However, few studies have examined whether this sensitivity may relate to heightened levels of chronic stress and persistently elevat...
OBJECTIVE: To explore how Michel de Montaigne's battle with urolithiasis influenced his writings and philosophical outlook during the Renaissance period. MATERIALS AND METHODS: The study examines historical texts, Montaigne's personal essays, and contemporary medical literature from the Renaissance era. A comprehensive review of Montaigne's extensive travels across Europe, interactions with prominent physicians, and the various treatments he sought for urolithiasis is conducted. The analysis ...
Recurrent hamstring injuries are highly prevalent amongst sporting populations. It has been hypothesised that pain from an initial hamstring injury may induce reorganisation of somatosensory representations that could contribute to reinjury. However, because of the cross-sectional nature of existing research, it remains unknown whether somatosensory changes are a cause or effect of pain or if they are driven by other potentially confounding factors. Here, we explored the effect of experimenta...
Objective: Prevention programs for eating disorders (EDs) and high body index mass (BMI) have the potential to reduce the onset of these interconnected public health concerns. However, it remains unclear whether routine implementation of such programs would be cost-effective. This study aims to determine the cost-effectiveness of an intervention that aims to prevent both ED and high BMI. Method: A Markov model was developed to evaluate the incremental cost-effectiveness of a targeted school-b...
In this global phase 2 study in patients with relapsed/refractory follicular lymphoma (FL), zandelisib was administered on intermittent dosing to mitigate immune-related adverse events and infections that have been reported with oral PI3Kδ inhibitors administered daily continuously. Eligible patients with measurable disease and progression after at least two prior therapies were administered zandelisib until disease progression or intolerability. The primary efficacy endpoint was objective re...
House dust mite (HDM) allergen immunotherapy (AIT) has an established role in the treatment of perennial allergic rhinitis (AR) and allergic asthma (AA) triggered by HDM sensitization. We aimed to identify all double-blind, randomized, placebo-controlled trials of HDM AIT for the treatment of AR and AA in humans and to summarize the evidence for AIT products that are currently manufactured and available for clinical use. A total of 56 eligible double-blind, randomized, placebo-controlled tria...
